Comprehensive Care for Our Community
Case Management & Life Navigation Services
Sojourners Outreach Services provides individualized case management to people experiencing instability. Case managers conduct intake assessments, identify barriers to stability, develop participant-centered action plans, and coordinate support services. Ongoing follow-up, goal tracking, accountability, and advocacy help participants move toward independence and long-term well-being.
Benefits Access & Public Assistance Navigation
SOS assists individuals in understanding, applying for, and maintaining access to public benefits such as SNAP (EBT), Medicaid, insurance programs, disability-related referrals, and other assistance programs. Services include application support, document gathering, appointment coordination, and follow-up with agencies to reduce barriers and delays.
Service Coordination & Referral Support
SOS connects participants to appropriate community services based on individual needs. This may include referrals to housing programs, healthcare providers, counseling services, workforce programs, educational opportunities, faith communities, and other nonprofit or public resources. Referrals are made based on participant choice, availability, and suitability.
Employment Readiness & Workforce Preparation
SOS provides job readiness support designed to prepare participants for sustainable employment. Services include resume development, interview coaching, workplace skills training, financial literacy education, soft skills development, volunteer placement, and connection to employment opportunities. SOS does not operate as a staffing agency but prepares individuals for successful workforce participation.
Entrepreneurship Education & Small Business Readiness
SOS offers educational programming and mentorship for individuals interested in self-employment or small business development. Services include basic business education, budgeting, planning, goal setting, mentorship, and referrals to external business development resources. SOS does not invest in, own, or operate participant businesses.
Emergency & Basic Needs Assistance
SOS provides limited, needs-based assistance to address immediate barriers to stability. Assistance may include food, clothing, hygiene supplies, transportation support, utility assistance, emergency repairs, or short-term stabilization aid. Assistance is provided pursuant to written policies, documented need, and case plan goals, with payments made directly to vendors whenever possible.
Short-Term Supportive Housing & Services Assistance
SOS may provide temporary financial assistance to help participants access housing or essential services during periods of transition or crisis. This may include short-term support for housing or care services while longer-term solutions (such as employment, benefits, or permanent housing) are established. Assistance is time-limited, documented, and tied to a stabilization plan.
Faith-Based Support, Mentoring & Community Integration
SOS offers voluntary faith-based support including prayer, mentoring relationships, Bible studies, church connections, and community engagement opportunities. Participation in spiritual activities is optional and never required to receive assistance. These services support emotional, spiritual, and relational well-being.
Transitional Employment & Skill-Building Opportunities
SOS may offer limited, time-bound transitional work or volunteer opportunities designed to build skills, confidence, and work experience. These opportunities are structured as training or rehabilitation activities and are not intended to replace permanent employment or serve as labor for unrelated business operations.
Advocacy & Ongoing Support
SOS advocates on behalf of participants when appropriate, assisting with communication, system navigation, and problem-solving across housing, employment, healthcare, and social services. Support continues as needed to help participants sustain progress and prevent recurrence of crisis.
